Apply

    Applicants must be sponsored by a sponsor/employer.  The applicant must fill
    out the application and the sponsor/employer must be listed on the
    application.  Once the application is submitted you will be contacted on whether
    or not you were accepted.  At that point if you were accepted then you and
    your sponsor will pay the amount due.

    Applications can either be printed and mailed in or filled out online and sent
